In reviewing your report there are numerous errors, my medications arenot correct and I gave the girl a list of them ? smoking status says occasionally smokes and I haven't had a cigarette in over 5 YEARS , I gave the receptionist my new insurance card and yet the girl charged me $69 copay when it should be $50 , please correct the report and mail me a new one
